Specialist Pharmacist to Support Transitional Care Ward in St Davids Hospital  (Pharmacy base - University Hospital Llandough).

We have an exciting opportunity for a motivated, enthusiastic and experienced clinical pharmacist to join our patient-focused and progressive  Pharmacy Team to develop this new role.

You will have the opportunity to set up and lead the new pharmacy service for the transitional care ward and play an integral role in directing the medicines service as part of the MDT. The post holder will have responsibility for managing and prescribing medication, including discharges, as part of the MDT, while ensuring that a sustainable, high quality clinical service that meets national standards and best practice is provided for our population.

Our aim is to help and support you set up and develop the new clinical pharmacy services to this speciality. Our innovative services, backed by strong technical support, are highly respected & fully integrated within the Hospital. Your professionalism, excellent inter-personal skills & enthusiasm will equip you to be a valued contributor to our service & a key member of our supportive clinical pharmacy team.
